About this service

Intensive specialist home physiotherapy

Some recoveries call for more than a weekly appointment. Intensive specialist home physiotherapy is a structured block of several visits a week, planned around clear goals, for people with complex rehabilitation needs — after a stroke or other neurological event, major orthopaedic surgery, or a long illness that has left them weak and deconditioned.

Frequent sessions at home allow more supervised practice, close monitoring and quick changes to the plan, without the strain of repeated journeys. How often Nasir visits is agreed with you after assessment and reviewed regularly, and family members stay involved so practice continues between sessions.

What's included in every visit

  • Visit frequency agreed with you and reviewed regularly
  • Weekly outcome review with family
  • Clear discharge or step-down plan

Who this helps

  • People with significant goals after a stroke or other neurological condition
  • Those in the later stages of recovery from orthopaedic surgery or a complex fracture
  • People who have become deconditioned after a long hospital stay or serious illness, including COVID-19
  • People living with Parkinson's or MS who want a focused period of rehabilitation
  • Families looking for a structured, time-limited programme with clear review points

What happens on a visit

  1. 1

    In-depth assessment and goal setting

A detailed assessment sets a baseline and identifies the goals that matter most to you. Together we decide whether an intensive block is appropriate and how often visits should be.

  2. 2

    Structured, frequent sessions

Each visit builds on the last, combining task practice, strength, balance, walking and endurance work, with the intensity matched to your energy and medical needs.

  3. 3

    Regular reviews with family

Progress towards your goals is reviewed with you and your family, and the plan is adjusted, up or down, depending on how you are responding.

  4. 4

    Step-down or discharge plan

Before the block ends you'll have a clear next step: fewer visits, an independent programme, or links back to NHS or community services.

Questions about intensive specialist home physiotherapy

How many visits a week will I need?

There's no fixed number. Frequency is agreed after your assessment, based on your goals, energy levels and other commitments, and it is reviewed as you go.

What if I get very tired?

Fatigue is common after stroke, illness and surgery, and pushing through it isn't always helpful. Sessions are paced, rest is built in, and the plan is eased back if fatigue or other symptoms worsen after activity.

Can intensive physiotherapy run alongside NHS therapy?

Yes. It is designed to complement NHS rehabilitation, not replace it. With your consent, Nasir can share updates with your NHS team so everyone works towards the same goals.
